"Just Get It Done" CPD Mentoring Agreement
with 4Front Pharmacy Solutions Ltd
This agreement is being entered into between Rachel Dungan of 4Front Pharmacy Solutions and $FIRSTNAME$ $LASTNAME$. Listed below are the expectations and terms of this programme agreement.
4Front is deeply committed to assisting pharmacist-led teams to transform their impact, at the heart of your community, through coaching and training, so that pharmacists are recognised as the healthcare professional of choice for medicines, health and wellness products, services and advice.
Supporting you to engage with MEANINGFUL Continuous Professional Development (CPD), which truly integrates your personal, professional, and business leadership growth is a power way to achieve this aim. When used optimally, the IIOP ePortfolio is a tool that provides the structure and space
- to evaluate what is working and what is not working as you integrate your personal, professional and business life
- to develop and use values-based decision making skills
- to prioritise and focus on what is most important
- to support follow up and accountability
- to enable you to demonstrate how you have transferred your learning into practice which drives personal, professional and business results.

A single 1.5-2hr intensive one-to-one mentoring session over Zoom, completely tailored to supporting you to get YOUR unique CPD

This programme is a one-off support to kick-start your engagement with an ONGOING process. 4Front's goal is to support you to attain your personal, professional, business leadership goals, as you and your team to continually develop the knowledge, skills, attitudes and behaviours to move from transactional to transformational leadership and consultations at the pharmacy counter.
As your CPD Mentor, you can expect 4Front to:
- Be your partner in assisting you to implement your structured, ongoing development plan to develop your knowledge, skills, attitudes and behaviours to move you forward, personally, professionally and in business.
- Provide the support to assist you to use the IIOP ePortfolio platform effectively to record your CPD
- Provide safety, encouragement and support; an environment in which you can engage in learning
- Provide access to relevant CPD webinars to support you as required.
- Respect the confidentiality of the agreements we make
- Expand your view of what is possible and promote discovery of new insights
- Give you input, straight feedback and operate as a sounding board
- Listen carefully to what you say and ask questions that increase awareness
Your Responsibilities, as 4Front’s Client include:
- Prioritise this investment in yourself
- Take personal responsibility for asking for what you need in order to achieve the results you want to achieve
- Prepare for our one-to-one session in advance by
- accessing your IIOP ePortfolio using your login details
- creating possible options for the CPD that you want to record
- experimenting with creating draft CPD cycles and
- reading the IIOP ePortfolio Review Resources.
- Based on your experience with the preparation tasks above, complete the pre-session prep form to include where you got stuck and the questions you want to have answered in our one-to-one session, so we can optimise the value from our time together
- Allocate time in advance to complete agreed actions post-session
- Provide feedback to 4Front regarding your experience and results
- Payment to 4Front at time of booking
HOW MUCH TIME SHOULD BE ALLOCATED?
Participants typically spend the following amount of time to successfully prepare for ePortfolio Review. With focused attention, you can be fully prepared in less than 1 day.
- 60 minutes to draft an OUTLINE of 5-10 CPD cycles
- 90 minutes for our one-to-one session
- Up to 180 minutes for post-session completion of tasks
WHAT SUPPORT WILL BE PROVIDED TO USE THE ONLINE PORTAL?
PRIVATE CLIENT ACCESS
Everything we will be using to support you (recordings, forms, appointment reminders, schedules, documents, resources, etc) will all be housed conveniently on a secured and confidential “Private Client Website”. Upon enrolling, you will be given a unique username and password and instant access to enrol for the live 'Preparing for ePortfolio Review' webinar (or recording depending on your schedule)
You will have convenient access to your “Private Client Website” area to access, review and record your questions and answers.
RECORDINGS
Your one-to-one CPD mentoring session may be recorded. Such recordings are made for training and development purposes only. In this case, you will also be provided with a copy of your session recording within the client log within 24 hours of the completion of your session. If you would prefer that your session is not recorded, you may also advise me accordingly.
PROBLEMS
If I as your coach ever say or do anything in a session that you don’t feel comfortable with or if you have any concern with the way we are working, please let me know as soon as possible. For our work together to be effective, it will rely upon you being honest and open in the best way we can.
CHANGES
In the case of emergency, you may re-scheduling your one-to-one appointment ONCE (at least 48 hours before planned date). Rescheduling more than once or giving less than 48 hours notice, without prior agreement with 4Front means that the missed session will be counted as a completed session and billed accordingly.
CONFIDENTIALITY
4Front represents, warrants and agrees as follows:
All information provided by you to 4Front during the course of CPD Mentoring will be deemed “Confidential Information” and kept strictly confidential unless otherwise agreed in writing. 4Front will not disclose any communications, either verbal or written, that have taken place either between 4Front and the Client or with regard to this Agreement without your prior written consent.
This confidentiality restriction also applies to testimonials, which 4Front may use for promotional reasons only with your consent. You may request anonymity in connection with any testimonials to be used.
Notwithstanding the above, “Confidential Information” does not include information that is:
- previously known to 4Front prior to discussions regarding this Agreement, free from any obligation to keep it confidential, or
- publicly disclosed by the Client either prior to or subsequent to the receipt by 4Front of such information, or
- independently developed by 4Front without any access to Confidential Information, or
- rightfully obtained by 4Front from a third party lawfully in possession of Confidential Information who is not bound by confidentiality obligations to the Client.
4Front may be required by law to disclose Confidential Information. If this is the case, 4Front, where reasonably practicable, and to the extent legally permissible, will provide you with prior written notice of the required disclosure.
To the extent legally permissible, at your written request, 4Front will delete all copies of the Confidential Information in its possession or control upon termination of this Agreement.
4Front Pharmacists, Trainers and Coaches are bound to honour both the Code of Conduct for Pharmacists and/or the International Coaching Federation Code of Ethics. For further information, please consult the links below.

DATA PRIVACY POLICY

Access to programme resources is granted, under contract, to paid up clients only. Unauthorised access is a breach of contract.
MAINTAINING RESULTS POST-SESSION
Achieving a world class practice of continuous professional growth and development is an ongoing process.
At the end of this agreement, you commit to either continue your engagement with 4Front as your coaching/training partner or to continue to maintain momentum, accountability and focus on the learning and development journey using internal resources.
You recognise that progress made and results achieved by engaging with CPD Mentoring will gradually dwindle and eventually fade away without continued focus and attention. You commit to accurately acknowledging the benefits and value of engaging in this mentoring process.
You commit to owning responsibility for any dwindling of results if you choose to shift your focus elsewhere post- programme completion.
